Dear Client,

Please confirm your home address, email address and cell phone number. All statements of account will be sent to your email and/or your home address.

Please put the date of your visit(s) on your payment check or I do accept Zelle banking transfers. Please send to 732-693-3080. I do not accept credit cards or Venmo currently.

Please check with your insurance company regarding your mental health benefits in and out of network. I am not an “in network provider.” I do not accept any Medicare, Medicaid or United Healthcare plans.

Some insurance companies are phasing out “out of network benefits” or limiting the number of sessions authorized (pay) to an out of network provider. It is the patient’s responsibility to ask me or your insurance company these questions before we begin our therapeutic relationship.

Those of you who have out of network benefits and would like us to submit a claim to your insurance company, except for Medicare, Medicaid or United Healthcare make sure we have updated insurance information.

Like any information shared on the internet via email, I cannot guarantee a 100% secure site, but I delete confidential information shared via email after it is printed. It is then placed in your individual hard copy chart. No client charts are filed online.

For appointment scheduling or conversations with me call my office, unless otherwise discussed. I do not check emails daily, but I do respond to phone calls within 24 hours or less. Texting is also a limited form of communication due to the high volume of text messages I receive daily. I cannot guarantee confidentiality or a secure site. No client’s name or number is “saved” in my business cell phone, unless otherwise discussed.

I do offer in-office sessions unless there is an uptick in Covid cases. Virtual tele-health sessions are always available. Therapy sessions not in-office are done on the phone, via FaceTime, or through telemedicine via doxy.me. Please send me an email or mail a hard copy note to my business address stating your decision to participate in therapy using one or all of these forms of communication.
