Insurance

Insurance

Dr. Jean Hager does not participate in any insurance networks and is not an in-network provider. She does not accept:

  • Medicare
  • Medicaid
  • United Healthcare
  • HMOs or managed care plans

Please check with your insurance company regarding your out-of-network mental health benefits before starting treatment. Some plans limit or do not offer out-of-network coverage.

If you have out-of-network benefits and would like a claim submitted on your behalf (excluding Medicare, Medicaid, or United Healthcare), please provide updated insurance information.

Medicare clients paying privately must sign a required opt-out agreement, per federal regulations.

Cancellation Policy

Your appointment time is reserved exclusively for you.

  • A 24-hour notice is required for cancellations.
  • Late cancellations (less than 24 hours) or no-shows will be charged the full session fee, except in cases of medical emergency or urgent unforeseen circumstances.

  • Please confirm your current home address, email, and cell phone number.
  • Statements of account are sent by email and/or mail.
  • Include the date of service on your check or Zelle payment.

  • Call the office for scheduling or clinical matters.
  • Phone calls are returned within 24 hours or less.
  • Email and texting are limited forms of communication and are not checked daily.
  • Confidentiality of email/text cannot be guaranteed.
  • No client names or numbers are stored in the business cell phone unless discussed.

No client records are stored online. Email communications containing confidential information are printed and placed in your paper chart, then deleted.

In-office sessions are available (unless public health conditions require changes).

Virtual sessions are always available via:

Please notify the office in writing (email or mailed note) if you choose telehealth services.

Email and text communication carry inherent privacy risks. If you are concerned about confidentiality, please avoid using these methods. There are inherent confidentiality risks in communicating by email and text. While safeguards are in place to protect your privacy, you should not use email or texting communication if you are concerned about any breaches of privacy that might inadvertently occur. Please contact the office with any questions.
